Did you know that your physician’s office is not responsible for notifying Public Health when your child receives an immunization?

Every time your child gets immunized you need to report it to your Regional Health Department.

You can view or report immunizations online using the Immunization Connect Ontario (ICON) tool. ICON allows you to report and view your child’s immunizations online any time. You can also attach a photo of the record.
