Annual Block Fee Plan
At Medical Associates of Port Perry, we’re committed to making your healthcare experience as simple and convenient as possible. To help with this, many family doctors are now offering an optional “Block Fee Plan”, which covers many common services that are not covered by the Ontario Health Insurance Plan (OHIP).
What is the Block Fee Plan?
The Block Fee Plan is an optional annual payment that covers many uninsured services for one full year. Instead of paying for each service individually when needed, you may choose to pay a one-time yearly fee.
Who can Enrol?
Families may enrol together under one plan, provided that:

- All members live at the same address (includes children 18 & under)
- All members are patients of the same family physician
Why consider enrolling?
Many patients choose the plan because it offers:

- Convenience – no need to pay each time you receive an uninsured service
- Predictable costs – one annual fee
- Peace of mind – access to common services when needed
What are Uninsured Services?
Some services are not covered by OHIP because they are considered non-medical or administrative. Some examples include:

- Sick notes or work/school notes
- Insurance, disability, and other forms
- Prescription renewals without a visit (when appropriate)
- Ear Flush (when not cover by OHIP)
In accordance with guidelines from the Ontario Medical Association, it is our policy to charge for these services. We have two options for you to cover the expense of any uninsured services you need. You can register for the optional “Block Fee” program, or you can pay on an as needed “Fee for Service” basis if not enrolled in the Block Fee Plan.
Whether or not you enroll, you will continue to have full access to medically necessary insured services.

- Enrollment in the Block Fee Plan is completely voluntary.
- Your decision will not impact your care or access to medical services.
- If you choose not to enroll, you may still request uninsured services and pay per service.
- The plan applies only to uninsured services and does not affect OHIP-covered care.
- Coverage with the block fee plan applies only to uninsured services provided by your family physician. Services provided by other physicians (e.g., walk-in clinics, urgent care, or hospitals) may still result in charges.
