The Port Perry Palliative Care Team is a multidisciplinary team that cares for patients and their families through the various stages of a terminal illness. We focus on compassionate care of the whole person, attending to their physical, emotional and spiritual needs.
The physicians of Medical Associates of Port Perry are dedicated to provide all patients in our area with compassionate, excellent end-of-life care. We are members of the Central East End-of-Life Network and the Durham Region Palliative Care Physicians Group. The same physicians care for people in the home, the clinic and the hospital, providing vital continuity of care. We work in liaison with the entire healthcare team, including nursing, allied health, chaplaincy, social work and the Community Access to Care Centre.
Most family physicians will continue to care for their patients through all stages of their illness. In some circumstances, they will consult with one of the physicians on the palliative care team for advice on advanced management of pain or other troublesome symptoms. Patients facing a terminal illness who don’t have a family physician in the area can be referred to the Palliative Care doctor on call at Medical Associates (call 905-985-2895 ext. 5417).
The team has a palliative care physician on-call 24 hours a day, 7 days a week, who can be contacted through the locating department at Lakeridge Health, 905-576-8711 (please be sure to ask for “The Palliative Care physician on-call for Port Perry”), Community Access to Care nurse or the Lakeridge Health Port Perry Emergency Department.
