North Durham COVID Assessment Centre

We are excited to announce the opening of the North Durham COVID-19 Assessment Centre on August 4th, 2020.
Located at the Prince Albert Hall at 19 Jeffery Street, Prince Albert, the clinic will complete COVID-19 assessments for residents of Scugog, Uxbridge and Brock Townships.

Community Response

The North Durham Family Health Team is taking special care when it comes to supporting seniors through COVID-19 pandemic. In addition to wellness and safety check in telephone calls, we are available to assist with prescription pick up and delivery and grocery shopping and delivery. To help pass the time, we have collected puzzles and crosswords, which we are happy to donate to seniors and those unable to get out of the house.  As a community, we will get through this crisis together.

What is a virtual visit?
  • Virtual visits are being performed via phone call and/or video visits.
  • Please ensure you have updated your phone number with staff when booking your appointment.
  • Video visits will require you to use a smartphone or computer with a camera and internet connection. If we feel that a video visit would be the best option for you, you will be sent an email invitation with instructions on how to attend the visit. Please ensure your email address is up to date and if you are not receiving the invitation as expected, please check your Junk folder.

What do I do if I have symptoms?
  • DO NOT come to the clinic if you are experiencing symptoms of COVID-19, such as Fever, Cough, or Shortness of Breath.
  • If you think you have coronavirus (COVID-19) symptoms or have been in close contact with someone who has it, use this self-assessment tool to help determine how to seek further care. https://covid-19.ontario.ca/self-assessment/
  • Lakeridge Health’s COVID-19 Assessment Centre provides screening and assessment for individuals in Durham Region who are experiencing symptoms of COVID-19. The COVID-19 Assessment Centre is by appointment only.
Should I wear a mask when I come to the clinic?

Please wear a non-medical mask while at the clinic.

What if I need a Prescription renewed?
  • Call your pharmacy and ask them to fax a prescription renewal request to your doctor 7-10 days before you are due to run out.
  • It is very important that you don’t wait until the last minute to do this as your doctor requires at least 48 to 72 business hours for all prescription repeats to be sent to your pharmacy.
  • Once your doctor receives this request, if he or she needs to book a virtual visit with you before repeating the prescription, one of our office staff will contact you to arrange an appointment.
I have an upcomng appointment booked, will it be cancelled?
  • Our team is working hard to reach all scheduled patients to either re-book or reschedule as a telephone or virtual appointment.
  • If you wish to cancel your upcoming appointment, please call our 24-hour cancellation line at 905-985-2895 ext. 6224.
I am due for a check-up. Can I book?
  • Please postpone any unnecessary appointments at this time.
  • Virtual Visits can be arranged with your health care provider if you feel you must be seen.
  • You may continue to book appointments by calling or booking online with some providers, however, please cooperate with COVID-19 screening. Please be honest and up front about your symptoms and exposure history so you do not put yourself or others at risk.
Is the Clinic an Assessment Site?
  • We DO NOT test for COVID-19 at the clinic
  • Lakeridge Health’s COVID-19 Assessment Centre provides screening and assessment for individuals in Durham Region who are experiencing symptoms of COVID-19. The COVID-19 Assessment Centre is by appointment only.
